The 1.25uf midrange cap is rated at 250VDC. I wonder if it needed to be rated that high (VDC) or if that rating was what was available at the best price to Klipsch at that time.
From what I remember, Klipsch was using components that had a +/- 20% variance so I'm sure cost was factor in there somewhere. Bob used Sonicaps (+/- 5% or less??) on mine which I'm sure cost quite a bit more.

As a short term fix try this: With the speakers turned OFF, turn the offending knob the full throw from low to high about 15-20 times. Turn the system back on and see if it any better. What this does is clean the carbon/dirt that's built up on the contacts in the dial pot of the volume control. Just a small note but I love the heck out of my P228 (13rd. 9mm). Bought it about 10 years ago NIB from a dealer for only $500. Wolfe springs and Pachmyer's is all that's been done to it. Seeing it's a "house" gun I don't want to do anything more rad to it for liability reasons.
Maybe the slide wasn't all the way forward.
Whenever racking the slide to load a new round into the firing chamber, let the force of the recoil spring do all the work. Meaning just let the slide go after you pull it all the way back. It's noisy but it's what the gun was designed to do.
